The player spawns in a desert, surrounded by rocky mountains. From these experiences, Cure#4 will be created, but will turn out to be a failure: instead of granting immortality as expected, the cure causes spontaneous combustion shortly after treatment, as the blood of a Nephilim makes a man burn. They extracted his eye and experimented on it, thinking that the secret of the Nephilim's immortality resided in their blood.
Resolving each riddle, they finally ended up by finding Immortius in his giant form, doomed to rot in chains for committing patricide and fratricide. After Bart_Montgomery has heard about immortal beings living in the desert, Ipiprix, the Head Doctor of the Department, will set up an expedition at the Nephilim Ruins, soon joined by Enrique_Graves. Through the exploration, the player will also be able to find the story that happened afterwards, concerning the search of the doctors in a cure for immortality. With this item, the player will be able to reach the last room where Immortius is chained, give him back the Eye and continue the story. If the player enters the game after completing “A L L” in the Mental Assistance Department game, the Eye of Immortius will be shown in their inventory. The puzzles are related to Immortius' backstory and the fall of the Nephilim, and other elements presented in the place give the player hints on the events that happened thousands of years ago. The game takes place in an unknown desert, where the player has to solve many riddles in order to progress throughout the place. In Ruina Eius de Nephilim, or The Fall of the Nephilim once translated, is a game created by Immortius.
